There is nothing new about the promise of integrated social care, and these days it is a driving force in government healthcare policy across Europe.

Reality, however, often falls short of expectation. But in Finland, a fast-maturing vision of truly integrated, ICT-enabled care delivery is showing the value of a collaborative approach.

The very word integration is taking on new layers of complexity as countries across Europe wrestle with the challenges of delivering dynamic networks of health and social care services, driven as much by political expediency as the rapid evolution of enabling technology. Where it was once about inter-connecting hospitals and mobilising clinicians and care providers, its focus today is on bringing health and social care – which have tended to exist in their own silos, colliding with each other rather than collaborating – into a cohesive, patient-centric delivery model.
